Compassionate Care at Home

End of Life Care at Home in Sydney

When someone is approaching the end of life, the help needed at home can change quickly. Young at Heart Services provides private end-of-life care at home in Sydney, supporting the person receiving care and the family around them.

For 18 years, we have helped people across Sydney live at home on their own terms. We bring that experience to personal care and everyday assistance, working alongside your doctors, community nurses and palliative care team.

Care can often start in 1 to 2 weeks

Care that respects your wishes

Your preferences remain central to your care. We take time to understand how you want to spend your day, who you would like involved and how we can help while respecting your privacy.

That may mean assistance with washing and dressing, quiet company while a family member rests, or help keeping familiar routines. Tell us about any cultural or religious preferences so these can be considered when arranging care.

Personal care and everyday help at home

Let us know about any equipment, mobility requirements or specific care instructions. This helps us understand the tasks involved and match the support to your needs.

Time for family carers to rest

When you are caring for someone close to you, another person’s help can give you time to sleep, attend an appointment or spend time together while someone else handles the practical tasks.

Respite can be arranged around the times you most need help. You can remain at home while a carer assists, or take time away with the agreed care in place.

Working alongside your medical team

Your GP, community nurses and specialist palliative care team guide medical care, including pain relief, symptom treatment and changes to medicines. Young at Heart provides personal care and practical assistance alongside that clinical support.

With your consent, we discuss relevant routines and care needs with the people already supporting you. If you have another home care provider, we can also discuss additional help to cover the times or tasks you need.

Arranging care before leaving hospital

If someone wishes to return home from hospital for end-of-life care, contact us while discharge is being planned. Tell us the proposed timing, the help needed at home and who is coordinating the clinical care.

The hospital team plans the clinical arrangements for discharge. We will explain the home care we can provide and when a suitable carer is available, so the practical support can be considered before the return home.

Keeping families informed and involved

Good care does not happen in isolation. With your agreement, we stay in regular contact with family and let them know about any changes we notice in day to day wellbeing, mobility or routine.

If we think some extra help would make life easier, or that a small change at home would help someone stay safe, we will say so. That might mean suggesting a handrail, a walker, a commode or another simple aid, or talking through whether a home modification is worth considering. Our aim is to keep communication open and to work alongside families, not around them.

Overnight and 24 hour care

You may need a carer at particular times of the day, through the night or for more continuous support. We can discuss overnight, live-in and 24-hour care, depending on the help required and carer availability.

If support is needed overnight, tell us what assistance is required and how often. We will discuss whether a carer needs to remain awake and confirm the hours, arrangements and costs before care starts.

Choosing your carer and discussing costs

We consider experience, personality and cultural or religious preferences when matching a carer. Our carers undergo reference and police checks and are covered by insurance. We welcome feedback and can discuss another match if you are not comfortable with the arrangement.

Fees depend on the care required and the agreed schedule. We discuss these with you before care begins. There are no administration fees, exit fees or long-term lock-in contracts. A notice period applies under your service agreement.
